WITHDRAWAL FORM
Sample form pursuant to EU Directive 2011/83/EU & §1820 of the Czech Civil Code
Use this form only if you wish to withdraw from the purchase contract. You may also communicate withdrawal in any other unambiguous way (e.g. by email to eshop@musoleum.cz), stating your order number and intent to return.
The withdrawal period is 14 days from the date you (or a third party other than the carrier indicated by you) take physical possession of the goods.

How to send
- —Email the completed form (or the same information) to eshop@musoleum.cz.
- —Or post the form to the address above.
- —Then return the goods, in original unused condition, to the same address. The Buyer bears the cost of return shipping.
- —Refunds are processed within 14 days of our receipt of the returned goods, using the original payment method.
